You haven't actually died, Are you running a custom plugin?
I have a custom plugin that shows on connected but instead of it saying you you killed yourself it says welcome to xx server

Delete these unneccessary plugins:

1. DataStoreSaver
2. IllegalName
3. AntiDizzy (WTF, that shouldn't even be there?)

I yesterday put plugin clan and get this bug too, but possibly this erorr get all server in one time, becouse my server this error get similar time, and server frieze

well if you know how to, You can open up a plugin and search it for

Player.SendCommand("deathscreen.show")

if you get any hits then thats the plugin causing it
