Players Appears With Strange0 Name and More Errors

Normal Players with Normal Names Like Example: [LifePlayer] magically change their name to Strange1, Strange2 Etc..
in certain Time the names is normal but certain period it changes to Strange1 , Strang2, Strange3
But Join Mensage tell Player :Example [LifePlayers] has Joined Server
when i type /players appear Strange1, Not Only one players its happened now i have 4 players with Strange on their name!
